the P4P Project
The Massachusetts
League of Community Health Centers is ramping up our pay for performance
capacity building initiative, ‘ABC’s of Community Health Centers:
Assuring quality, Balancing pay for performance and Continuing
to improve.’ We have
partnered with funders Blue Cross Blue Shield of Massachusetts, Partners
Healthcare System, and Neighborhood Health Plan to offer a full curriculum
of workshops, trainings, and events.
Based on a needs
assessment and a broad review of the external environment of P4P activities,
the following have been identified as critical issues to be addressed
through this project:

Development of a
strategic change leadership approach supporting improvement team models
and systems;
-
Identification and
addressing of gaps in self–perception of care provided compared to
evidence based and documented care and addressing health disparities;
-
Support for the design
of and resources needed to expand improvement systems to be used across
the center’s services, providers, and staff;
-
Strengthened
documentation of care, coding and capture of provider services within
billing and reporting functions;
-
Strengthened use of
existing internal data for management of expectations and outcomes
strategies of provider teams;
-
Enhancement of available
data, reports, information systems and clinical decision making tools to
support performance management and improvement initiatives;
-
Adoption and
sustainability of project best practices for continued success under pay
for performance and quality incentives.

Advanced
Provider Communication Workshop Series (3CPO)
The Center for
Clinical Communication and Performance Outcomes (3CPO) affiliated with
UMASS Medical School, in collaboration with the Massachusetts League of
Community Health Centers, will be conducting a series of training
workshops tailored to the needs of community health center primary care
providers designed to improve the quality of patient and provider
experiences in community health centers throughout Massachusetts.

Cultural
Competency
Neighborhood
Health Plan and the Harvard Pilgrim Health Center Foundation Institute
for Linguistic and Cultural Skills, in collaboration with the
Massachusetts League of Community Health Centers, will
offer Foundations in
Cross Cultural trainings for
Nurses, Behavior Health staff, Physicians, Nurse Practitioners and
Physician Assistants, Pharmacists and Dentists.

Financial
and Quality Readiness for Pay for Performance
This three-session
course is geared toward health center analysis of its own data and
workflow processes in preparation for pay for performance initiatives.
Development of the series was supported by a needs assessment survey of
selected health center Chief Financial officers, Executive Directors,
Quality Managers, network managers, and Medicaid managed care plans. The
target audience is pairs of Chief Financial Officers and Medical Directors
from the health centers.
Care Model
Orientation
Health center
staff attendance at a Care Model Orientation supports the integration
of the Care Model in your organization. Expand the model's application
beyond your clinical staff by including managerial staff in these
orientations. Recommended
audience:

Managerial
staff, including operations directors, practice managers, nurse
managers, quality improvement coordinators or directors,
administrative managers
-
Clinical
staff, including physicians, midlevels, nurses, medical
assistants, case managers, outreach workers

Senior
health center leadership
