Four Part Learning Collaborative

The League has engaged Health Management Associates and CohnReznick LLP with funding support from the Partnership for Community Health (Neighborhood Health Plan and Partners HealthCare) to provide a four-part ACO Readiness Learning Collaborative to assist community health centers in responding to MassHealth's Delivery System Restructuring.

This Learning Collaborative will provide a series of four full-day sessions for the health center leadership teams and managers in addressing key components of readiness and implementing actual participation in an ACO and developing ACO operations management systems. Presenters and facilitators include: Peter R. Epp, Linda Follenweider, Art Jones, MD, Myra Sessions, Lisa Whittemore and Deb Zahn. CHC C-Suite and ACO led team encouraged to participate.

Session #1 - May 19

  • Overview of Readiness: How to Succeed in the New Environment
    • Setting the Tone - Managing and sustaining change in teams, leadership, engagement, and incentives
    • Review of the Value Based Payment roadmap
    • Creating community partners and ACO models

  • Elements for Success - Financial and Quality Infrastructure for Accountability
    • Organizational Risk - Funds flow, infrastructure investments and levels of risk
    • ACO Negotiation Strategies - Health plans, payers and CHC positioning
    • Data Systems and Internal Reporting - MassHealth quality framework and total medical expense management
    • Quality and incentive model, compensation systems

  • ACO Risk Stratification and Coding for Improvement
